62 year old female with left breast metastatic carcinosis from prior esophagus cancer. Ultrasound of breast (3A) showing a hypoechoic lesion with irregular shape, angulated margins and macrocalcification (arrow), measuring 2.5 × 2.3 × 1.7cm. Lesion is seen to abut pectoralis major muscle but not infiltrate it (3B).. On color Doppler assessment of the breast lesion, internal vascularity (arrow) was noted.Assessment of the breast lesion on B-mode ultrasound (5A) and strain elastography (5B) revealed a hypoechoic lesion with angulated margins which is stiff as indicated by red color on the color coded map of strain elastography. The regions of interest (ROI) were selected in the preset shape of ellipse (arrows) and the fat to lesion strain ratio with respect to the ROIs was calculated to be 5.33.
