Hypomelanosis of Ito is a rare neurocutaneous syndrome , its is also known as incontinentia pigmentiachromians. It is characterized by hypopigmented skin patches/ streaks or mosaic pattern with hypopigmented and hyperpigmented patches . 6 year boy came for MRI with complains of delayed development , hypotonia , seizures. FLAIR images reveal hyperintensities in periventricular white matter. Multiple hypointense cysts are seen within this white matter . T2W images similar bright signal in periventricular white matter with cysts formation within affected white matter s/o dilated Virchow Robin spaces. Hypopigmented patches were seen in upper and lower limb.
