Primary appendiceal adenocarcinoma is a rare malignancy, which comprises less than 0.5% of all gastrointestinal neoplasms. Most commonly, these tumors present as acute appendicitis or as a right lower quadrant abdominal mass when associated with a mucocele. We report the unique case of mucinous appendiceal adenocarcinoma in a 57-year-old male who presented with a right upper thigh mass secondary to herniation of the neoplasm from the right lower quadrant through the inguinal canal. The axial images provided demonstrate a multiloculated cystic mass in the right lower quadrant which communicates with an extensive cystic lesion throughout the soft tissues of the right thigh.
