Category: Abdominal Imaging, Region: Abdomen-Liver, Plane: Axial
Technique: Axial pre-contrast enhanced T1W High-Resolution Isotropic Volume Examination (e-THRIVE) (3T, TE 1.39, TR 3) (Fig. 03a), axial T2-weighted Spectral Attenuated Inversion Recovery (SPAIR) (3T, TE 80, TR 1250) (Fig. 3b), and MRCP (3T, TE 800, TR 8000) (Fig. 3c). Findings: A 5.0 x 4.0 cm round lesion in the region of the CBD (short yellow arrows, Fig, 3a, 3b, 3c), in the vicinity of the duodenum, medial to the pancreatic head (pancreas outlined in yellow, Fig 3a). The lesion showed increased signal on T1-weighted images and a thick low-intensity signal rim on T2-weighted images in favor of accumulation of calcifications and proteinaceous material in a pre-existing choledochal cyst. MRCP (Fig 3c) shows the obstructing lesion (short yellow arrows) with dilated extra- (yellow asterisk) and intra-hepatic (long yellow arrows) bile ducts.
