A CASE OF PERFORATED SOLITARY CECAL DIVERTICULITIS ON COMPUTED TOMOGRAPHY IMAGING

The 43-year-old male patient presented to the emergency room due to 3 days of severe right lower quadrant abdominal pain. He complaint of fever, anorexia, nausea but no vomiting. Rebound tenderness was elicited in his right lower quadrant on physical examination. His white blood cell (WBC) count was measured at 19.8 with neutrophilia. Other hematology and biochemical parameters were unremarkable. Emergency Computed Tomography (CT) scan was performed and showed the appendix was normal. However, multiple right sided cecal diverticula were present with adjacent inflammatory fat stranding and pockets of free air. No diverticula were identified in the rest of the colon including the descending and sigmoid colon. Findings were consistent with Hinchey 2 acute solitary diverticulitis of the cecum. He was successfully treated with intravenous antibiotics and rehydration with resolution of his symptoms and scheduled for follow-up colonoscopy six weeks later.
